Did a search of the address on the packaging. Came up as a rental. Might try and contact the landlord and see if he left a forwarding address.
Found the listing on Zillow.

Assuming it's the same person, the last I've been able to find is that in 2015, he had moved to Arkansas. Can't find a name or e-mail though.
